Information collected

The registration form collects athlete and parent contact information, date of birth, address, emergency-contact information, schedule selections, relevant notes, and electronic waiver records.

How information is used

This information is used to organize training groups, communicate schedule information, respond during an emergency, document waiver acceptance, and administer the Oregon City Men’s Basketball Summer Training Program.

Who can access it

Registration records are available through Coach Daniel’s private administrator account. Families receive a private schedule-management link and should not share it.

Retention

Program registration and waiver records may be retained for up to five years after the program, unless a longer period is required to address a legal, insurance, or safety matter.
